The Registration Form is in Word Format so you can simply fill in the form and saved as YOUR NAME (ex. JUAN DELA CRUZ) and email back to us. Please scan your bank payment receipt and email (bsop.alumniassociation@gmail.com) together with the Registration Form.

You can also fax your Registration Form and bank payment receipt to BSOP Fax # +63 2 292-6675. Please indicate your name and add ATTENTION: JULIET LEE UYTANLET so that the faxed mail will be forwarded to BSOP AA.
